چکیده

Background Psychiatric comorbidities can be a significant additional burden in chronic diseases. The most common inflammatory rheumatic disease children and adolescents is juvenile idiopathic arthritis (JIA). Data on mental illness with JIA are heterogeneous. Objectives To assess the frequency of depressive anxious symptoms patients compared to healthy peers. Methods were analysed from controls same age included inception cohort newly diagnosed (ICON). Depressive (using Patient Health Questionnaire (PHQ-9, score 0-27) (Generalised Anxiety Disorder Scale (GAD-7, 0-21) captured 7 or 9 years after inclusion ICON aged thirteen older at time filling these questionnaires. Symptom severity for both instruments was assessed by sum following cut-off values: PHQ-9 < 5: none, 5-9: mild, 10-14: moderate, 15-19: severe, ≥ 20: very severe. GAD-7 Score 15: Disease parameters such as Physician Global Assessment Activity (PhGA Activity, numerical rating scale, (NRS),0-10, 0=best), joint count (n) patient-reported outcomes functional limitations ((C)HAQ, 0-3, Well-being (PGA Well-being), pain fatigue (NRS, 0-10, 0=best) also documented. Results analysis 344 patients, 157 (45.6%) 18 old (mean 15.5 ± 1.5 years, 64.3% female), 187 (54.4%) 21.5 2.1 65.2% female) 224 control subjects, 115 (51.3%) 15.2 60% 109 (48.7%) 21.4 1.9 58.7% female). Almost 40% had oligoarthritis (26% persistent OA, 12.5% extended OA), 27% rheumatoid factor (RF)-negative polyarthritis, 6% psoriatic arthritis, 17% enthesitis-related arthritis; 3% each systemic RF-positive polyarthritis. In total cohort, 14% 7% 10 10% 2% 10. Within categories JIA, rate ranged 9.3% (oligoarthritis extended) 33.3% (RF-positive polyarthritis) 0% (systemic arthritis) 22.2% (psoriatic arthritis). Patients higher scores (≥ 10: 18.7%) 14.4%) (PHQ-9 8.3%, 5.1). versus 10, there no differences either PhGA activity (0.8±1.6 / 1.0±2.0, p = 0.673) (0.5±1.3 0.5±1.6, 0.999). contrast, difference (0.8±1.5 1.6±1.4, 0.005) but not (0.7±3.1 0.8±1.3, 0.850) Female more often found have depression anxiety than male female 17.5%, 7.4%, 13.5%, 4.1%) 4.4%). proportion (13.5%) subjects (2.3%) remarkable, this (4.1%) only slightly (2.2%). Conclusion young adults especially females. continuous care standardised diagnostic tools should implemented detect comorbidities, optimise therapy thereby reduce disease. Further research needed identify possible predictors development order pursue preventive approaches.
